They must also be able to work under pressure and adapt to the unique challenges of long-term care settings Key Responsibilities of LPNs in LTC and SNF • Patient Care: provide essential care to residents, including monitoring vital signs, assisting with daily activities (such as bathing, dressing, and eating), and ensuring proper nutrition and hydration. • Medication Administration: administer prescribed medications and treatments, ensuring that residents adhere to their medication schedules. • Health Monitoring: regularly check on residents' health, observing any changes in their condition and reporting these to physicians. • Collaboration: work closely with RNs, physicians, and other healthcare professionals to develop and implement individualized care plans for residents. They also supervise Certified Nursing Assistants (CNAs) to ensure quality care delivery. • Emotional Support: Provide emotional support to residents and their families.
