The Children, Youth, and Family Mobile Crisis Team is a mobile crisis team that provides crisis services twenty-four seven, three hundred and sixty-five days a year. This is a unique and specialized team that aids children and youth in resolving crisis and providing for the engagement and coordination of community services specializing in engagement and treatment of children and youth. The team will provide rapid response, assessment services, crisis intervention, supportive counseling, linkage to requested/necessary resources and appropriate community-based mental health and SUD treatment. SHIFT(S) AVAILABLE: Eligible for added shift differential. • Monday-Friday 9PM-5:30AM - 9.63% The Crisis Clinician is a member of a multi-disciplinary team, which provides telephonic and outreach crisis intervention and evaluation services to individuals and families. The primary goals of these programs are to divert individuals from admission to an emergency department, the criminal justice system, and inpatient psychiatric treatment while maximizing the use of community resources for stabilization purposes. These programs serve individuals who could have a co-occurring disorder and may also be homeless. The Crisis Clinician provides rapid response, assessment services, crisis intervention, engagement, supportive counseling, information and referral, assistance with Medicaid enrollment as appropriate, linkage to requested/necessary resources and appropriate community-based mental health and linkage to substance use services for ongoing treatment and follow-up. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. • Provide initial and follow-up phone triage, assessment, and de-escalation services for individuals who are in an emotional or behavioral health crisis, including those referred for Involuntary Treatment Act (ITA) services. • Provide crisis and hospital diversion services to youth, adults, and families in a behavioral health crisis and facilitate use of/access to crisis hospital diversion resources. • Provide referral services to individuals requesting assistance or 3rd party referral sources to provide community stabilization and diversion services to the identified individual in crisis, including linkage to a Designated Crisis Responder (DCR). • Complete risk assessments to determine if the person is at risk for suicide and/or homicide, and staff the results of the assessment with a supervisor or clinician II to ensure the person's safety and the safety of others. • MRRCT staff will work in collaboration with the Mobile Dispatch Unit to ensure case response meets contract requirements. • High-risk cases must be brought to a supervisor, in a timely manner, for immediate and appropriate assistance related to response. • Provide initial and follow-up phone and face-to-face assessments for individuals who are at risk. • Create or further develop recovery plans and on-going support to promote stabilization plans with the individual's support network. • Refer individuals who are eligible to Trueblood forensic programs, as appropriate. • In collaboration with clients and their support network, develop stabilization and safety plans that are reviewed with a Clinician II or supervisor. • Complete comprehensive documentation which supports the safety plan as indicated. • Possess an in-depth knowledge of community resources and ability to access resources to support and address client needs. • Follow up on referrals made regarding at risk/high risk individuals to ensure stabilization and/or services obtained. • FBH will attempt to hire staff for a fixed shift. However, there may be a time that a clinician may be asked to work a different shift due to coverage concerns. • Maintain working relationships with the courts, attorneys, providers, police and sheriff's deputies, jail staff, clinical and administrative persons at inpatient psychiatric facilities throughout the state, staff of other agencies, advocates and consumer groups and citizens. • Participate in Peer Review, supervision, in-center educational and administrative meetings as directed. • Maintain client records and all other required documentation in a timely and confidential manner, in accordance with agency policies and practices. • Assist non-Medicaid individuals to access Medicaid as appropriate. • Provide case management and wrap-around services as needed. • Assist clients in obtaining basic needs such as emergency housing/shelter services, financial assistance, medical services, medications, and other resources that will help them maintain stability in the community. • Provide coaching, advocacy, information and referral, linkage and coordination with resources, and ongoing support. • Within a year staff will be proficient in best practice for children, youth, and families. • Requires travel to see clients in the community and travel between work sites either via FBH owned Vehicles and/or personal vehicle. • Other duties as assigned. QUALIFIED CANDIDATES WILL HAVE: REQUIRED: • Bachelor's degree in behavioral health or related field. • Must apply for a Counselor Agency Affiliated – Registered credential from the WA State Department of Health within 30 days of hire. • Must have an unrestricted and valid driver's license, be able to pass a motor vehicle screening and driving test, have active car insurance, and be willing to use their personal vehicle in the course of work. PREFERRED: • Two years of experience serving children, youth and families with behavioral health needs. KNOWLEDGE, SKILLS & ABILITIES: • Strong engagement and customer service skills, empathy, ability to remain calm, professional, and clear headed during escalated situations. • Knowledge and skills in working with individuals with mental health and/or substance use disorders. • Knowledge of or ability to gain knowledge of all WAC and RCW requirements for Crisis and Involuntary Treatment Services • Ability to communicate and engage effectively with individuals of varied backgrounds, trauma experiences, cultures, education level, and socio-economic status. • Ability to manage 10 to 20 active cases • Demonstrate evidence of critical thinking, decision-making ability, problem-solving skills and ability to work autonomously. PHYSICAL, SENSORY & ENVIRONMENTAL: • This position provides client services in community settings on a daily basis. • Community settings include but are not limited to residences, group/congregate living settings, hospitals, shelters, the streets or anywhere in the community at large. • Some environments may not be ideal in terms of cleanliness or being habitable. • Daily travel between locations is required. • The ability to drive a vehicle, traverse distances up to several city blocks and on various terrains, navigate stairs, get in and out of vehicles frequently is required. • Daily use of a computer is required. • May be required to lift 10 - 20 pounds. • Blood-borne Pathogen Category III - No predictable exposure to blood-borne pathogens PAY RANGE (Hourly/ Non-Exempt):$25.89- $31.53 • All pay is based on longevity at Frontier Behavioral Health, thus the starting base pay for this position is $25.89. • There will be added shift differential of 9.63% for the Monday- Friday 9PM - 5:30AM shift. The Providence Spokane Heart Institute and Center for Advanced Heart Disease and Transplantation is expanding and seeking a full-time BC/BE Advanced Heart Failure and Transplant cardiologist at 0.8 FTE with a 0.2 FTE Medical Directorship. Join a team of heart failure cardiologists and nurse practitioners continuing the tradition of over 30 years in heart transplantation in the Inland Northwest. This is a comprehensive service line encompassing ambulatory advanced heart failure and transplant clinics with specialty areas in amyloidosis, sarcoidosis, cardio-oncology and hypertrophic cardiomyopathy. Providence Sacred Heart Hospital is quaternary medical center serving a large geographic region of Eastern Washington, Idaho, Montana and Eastern Oregon. The heart failure program runs a dedicated inpatient service caring for the full spectrum of heart failure patients including heart transplant, LVAD, percutaneous mechanical support, ECMO, TAH, and cardiogenic shock.
