The position requires timely documentation and communication, proficiency with Homecare Homebase (HCHB), and OASIS experience. Job Details • Schedule: Monday through Friday, 8-hour day shifts; weekends are optional. Minimum of 40 hours per week. • Required Qualifications: Active physical therapy license in hand at submission; Basic Life Support (BLS) certification; minimum of 1 year of recent relevant experience; OASIS experience; proficiency with Homecare Homebase (HCHB); active drivers license and personal transportation; flexibility with the assigned territory; and timely documentation and communication skills. • Additional Information: • Home health experience serving geriatric patients. • On-call rotation is required approximately every sixth weekend. • First-time travelers are not accepted. • Monday orientation is required. • Scrubs may be any color; jeans are not permitted. • Requested time off must be submitted at the time of application. • A home health checklist is required. • Orientation modules are completed during orientation.
