Job Details • Schedule: Day shift, 8 hours per shift, with a minimum of 40 hours per week. • Required Qualifications: • Minimum of 1 year of recent relevant experience • Basic Life Support (BLS) certification • National Board for Certification in Occupational Therapy (NBCOT) certification • Occupational therapy license in hand at the time of submission • Outcome and Assessment Information Set (OASIS) experience for staff completing Start of Care (SOC) home health visits • Homecare Homebase (HCHB) proficiency • Active drivers license and personal transportation • Previous travel experience required; first-time travelers are not accepted • Additional Information: • Provides care primarily to geriatric patients in a home health environment. • Average daily caseload is 57 patients. • On-call rotation is required every sixth weekend. • Timely documentation and communication are mandatory. • Flexibility with the assigned service territory is required. • Monday orientation is scheduled. • Scrubs may be any color; jeans are not permitted. • An iPad is provided for work-related use. • Requested time off must be submitted at the time of application. • A home health checklist is required. • Travel pairs may be accepted, but each traveler is assigned an individual territory.
