Details • Home health setting covering Osceola County, split into east and west coverage areas • Monday-Friday schedule, 8:00 AM-5:00 PM, 40 hours per week guaranteed • Average of 35 miles driven daily between patient visits • PT caseload averages 3-4 visits daily including OASIS-type visits • PTA caseload averages 6 patients daily at minimum Requirements • License required if applicable • One professional reference covering entire work history required • Social Security Number and Date of Birth required for consideration • COVID vaccination card or approved declination required Additional Information • Must comply with facility's internal call-off policy, which may result in rescheduling to another weekend or holiday shift if a weekend or holiday shift is missed • Local candidates accepted at the same pay rate as traveler rate with no radius restriction • Permanent employees must be separated from the facility for 90 days before returning as a traveler • Two doses of COVID-19 vaccine or an approved declination required; booster not currently required American Traveler Job ID #P-779825.
