This is a PRN position, a caregiver in this position works variable, flexible hours. A caregiver who excels in this role will: • Ensure continuity of care and coordinate discharge planning. • Act as an interdisciplinary team member by providing direct patient care under multiple levels of care. • Collect and document objective and subjective patient data, in accordance with the agency policies and procedures. • Report data, as appropriate, to the directing registered nurse and other members of the health care team, so that a plan of care is initiated, and changes are accurately addressed. • Actively engage with and educate potential customers about the benefits and appropriateness of care. • Ensure the patient and their family have adequate knowledge and an understanding of the hospice or homecare benefit, as appropriate. • Contribute to the assessment of patients by collecting data for analysis by an RN. • Observe patients and report any unusual conditions to professional staff. • Observe patients for safety risks and implement safety measures that are within scope of practice. • Administer medications, within scope of practice, and observe patients for effectiveness in relief of symptoms as well as for adverse reactions to medications or treatments. • Provide direct patient care, including dressing changes and laboratory tests, as part of the plan of care. • Provide ongoing education and information to patients and family members, taking into consideration cultural issues and other special challenges. • Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions. • Ensure a seamless transition to the residential setting by coordinating services at time of discharge. • Be flexible in setting and revising priorities for one's daily assignment based on patient needs/acuity, family requests and available resources. Minimum qualifications for the ideal future caregiver include: • Graduate from an accredited Practical or Vocational Nursing Program • Current Ohio license as a Licensed Practical Nurse (LPN) • One year of clinical experience as a Professional Nurse in an acute setting. • Basic Life Support (BLS) certification through the American Heart Association (AHA) or the American Red Cross • Valid Ohio driver's license and proof of automobile insurance coverage with $100,000/$300,000 coverage • Knowledge of JCAHO and Medicare standards and regulations Preferred qualifications for the ideal future caregiver include: • Infusion experience • Hospice experience Physical Requirements: • Requires full range of motion, manual and finger dexterity and eye-hand coordination. • Requires standing and walking for extensive periods of time. • Requires corrected hearing and vision to normal range. • Requires some exposure to communicable disease or bodily fluids. • Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. • Physical Demand requirements are in excess of those for Light Work. • Must be ambulatory and able to drive to patients' homes, able to climb stairs, pull, push, and facilitate movement of the patient. • Must be able to read, write and communicate with patients. Personal Protective Equipment: • Follows standard precautions using personal protective equipment as required. The Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) will provide processing and communication of Commercial Prior Authorizations (PA) reviewed by the Commercial PA team for Pharmacy benefits. This is a high production role with required metrics. Position Summary: The Licensed Practical Nurse (LPN) will provide processing and communication of Commercial Prior Authorizations (PA) reviewed by the Commercial PA team for Pharmacy benefits. Reporting to the Pharmacy Operations Manager, working with physician office staff and customer service, admissions, and pharmacy operations departments to communicate referral status. You will also oversee fax processing and providing telephone call assistance for prescriber office staff, pharmacies and members based on program criteria. Responsibilities include answering inbound phone calls, processing cases, loading authorizations, and making follow-up phone calls. Maintaining complete and accurate, documentation of all necessary information is necessary and will involve computer system data entry, data management, and reporting. Duties: • Review criteria-based prior authorizations following policy and procedure • Provide internal nurse support to the PA team and member services department Refer cases not meeting clinical criteria to the Pharmacist and MD team(s). • Shift priorities while exhibiting a high level of urgency with all calls and assignments. • Work with pharmacists, other clinical colleagues, healthcare professionals, and members. • Follow all prior authorization procedures to guarantee an accurate process on each prior authorization. • Complete other PA assignments as delegated by the PA leadership team. • Utilization of clinical skills to support the documentation and communication of medical services and prior authorization determinations • Ability to multitask, prioritize, and effectively adapt to a fast-paced changing environment • This position involves sedentary work with periods of sitting, talking, listening. • Work requires sitting for extended periods, talking on the telephone, and typing on the computer. • Position requires proficiency with computer skills which include navigating multiple systems and keyboarding • Excellent communication skills, both written and verbal • Must have regular and predictable attendance • Performs and handles inbound and outbound phone calls with technicians, prior authorization team members, physicians, healthcare providers, and/or patients to facilitate prior authorization requests, answer inquiries, and/or resolve calls with a one call solution. • Performs other related projects and duties as assigned, including attending training sessions and development meetings. Experience: • Minimum 2 years clinical experience as an LPN/LVN. • A LVN (Licensed Vocational Nurse) or LPN (Licensed Practical Nurse) that must hold an unrestricted license in their state of residence or a compact license. • Prior authorization experience or Case Management experience • 3+ years prior related clinical work experience • Ability to prioritize, quickly assess, manage multiple tasks, and adapt to constantly changing situations. • Experience using MS Office and other Windows-based computer applications and phone use. • Digital charting • Managed care or health plan previous experience, • Call center experience • Multi-tasking experience • Prior remote experience preferred • Case Management experience preferred • Prior authorization experience • High production experience • Excellent organizational skills. Education: LVN/LPN Diploma Associate's degree in nursing as LPN/LVN Anticipated Weekly Hours: 40 Time Type: Full time Pay Range: $21.10 - $53.53 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
