The position provides in-home rehabilitation services to geriatric patients, supporting mobility, recovery, and functional independence. The role requires timely documentation, effective communication, and flexibility in serving patients across the assigned territory. Job Details • Schedule: Day shift, 8 hours per shift, 40 hours per week. • Required Qualifications: • One year of recent relevant experience. • Basic Life Support (BLS) certification. • Active professional license in hand at submission. • Active drivers license and personal transportation. • Proficiency in Homecare Homebase documentation. • OASIS experience required for clinicians completing Start of Care visits. • First-time travelers are not accepted. • Preferred Qualifications: Experience treating geriatric patients and individuals with orthopedic, neurological, spinal, post-surgical, and cognitive conditions. • Additional Information: • Typical caseload averages 57 patients per day. • Productivity expectation includes six daily visits or a minimum of 30 points weekly. • Timely documentation and communication are mandatory. • On-call rotation may be required. • Scrubs may be any color; jeans are not permitted. • An iPad is provided for clinical documentation. • Orientation is scheduled for Monday. • Requested time off must be submitted at the time of application. • A Home Health checklist is required.
