Design and evolve individualized care plans that align with the master treatment goals, ensuring every intervention is purposeful and responsive to the patient's progress. Lead therapeutic engagement by co-facilitating group therapy sessions and implementing evidence-based nursing care that helps patients develop vital coping and stabilization skills. Collaborate as a clinical advocate within multidisciplinary team conferences, providing critical insights that shape the goals and objectives of the patient's long-term treatment plan. Optimize treatment outcomes by continuously evaluating patient responses to care and proactively adjusting interventions to meet their changing clinical needs. What minimum requirements you'll need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Licensed Registered Nurse credentialed from the Kansas Board of Nursing required. Education: Diploma from an accredited school/college of nursing OR professional licensure at time of hire required. Position Overview The Home Care RN is responsible for providing skilled nursing services, supervising patient care, and ensuring compliance with physician orders, state regulations, and agency standards.
